The difference is that to become a doctor you NEED to go through several years worth of school and get the grades. To become an actor you can literally just know the right person. Hence nepotism.
if your parents are doctors they might have money for tutors and private schools and good colleges that can help you also become a doctor. much like how growing up around film sets and having a familial network of agents and directors at your disposal might help a budding actor
